(AMATEUR SWIMMING ASSOCIATION)

Newsletter detailing affiliation to ASA which was distributed during
December and is displayed on the Club notice board
| WHAT DO I NEED TO DO? |
| Please collect a pack of forms from White
Dolphins' pay desk set up on club nights in the rear Hobart entrance.

| WHO COMPLETES THE FORMS? |
| Forms can be completed at club or home;
signatures from both parents and swimmers are required (providing the
swimmer is old enough to write their name.) Please note the green ASA
Registration Form should be completed in black ink. |
| WHAT HAPPENS TO COMPLETED FORMS? |
| Please return completed forms to club with
(where possible) a cheque payable to 'Loddon White Dolphin Swimming
Club'. |
| HOW MUCH DOES AFFILIATION COST? |
| Cost of ASA affiliation is £11.05 per
child per annum and includes insurance; the club does not receive any
part of this amount. |
| MY CHILD SWIMS AT ANOTHER ASA AFFILIATED
SWIM CLUB AS WELL AS WHITE DOLPHINS; DO I HAVE TO PAY TWICE? |
| It is unlikely. You do still need to complete
an ASA/White Dolphins affiliation pack. Please ask your other swimming
club for their ASA club code - usually 4 letters - and annotate under
'Club Code' on the green Registration Form. |

| When you bring your child to club please take a
minute to read the Photography Policy displayed at the club pay desk in
the rear Hobart entrance. |
| ASA MEMBERSHIP CARD? |
| Swimmers will receive an ASA membership card -
through the club - once the registration process is complete. |
| WHAT DO I KEEP? |
| Please retain back 3 sheets of pack for
your reference - |
